CORE’s clinical decision-making tools enable individuals to develop person-centered treatment plans that help them live independent lives in the communities of their choice.

Our unique approach facilitates meaningful conversations between professionals and clients, deepening the understanding of mental health and wellness. This empowers individuals to clearly define how their mental health affects their lives and goals. During discussion, clients identify barriers preventing them from achieving their goals.  

CORE’s innovative system categorizes barriers into Barrier Groups, which are tailored to specific mental health conditions. These Barrier Groups help individuals articulate their challenges, prioritize their needs, and plan for services that best support their well-being.

By selecting Barrier Groups, individuals gain access to curated lists of skill-building activities. Individuals can then choose activities that align with their preferences and goals. CORE’s activities are designed to leverage existing strengths, develop new skills, and ultimately help individuals overcome barriers that have stood in the way of achieving their goals.

The final step in this clinical decision-making process is recording the selected activities on the individual’s person-centered plan. CORE offers two distinct planning resources: the Course of Recovery and the Wellness Plan.

course of recovery

This resource helps the individual build an accomplishment driven life by providing a framework for identifying goals and useful, rehabilitative activities. The Course of Recovery records the individual’s diagnosis and their unique symptoms, strengths, and goals. Designed as a quick reference, it allows the individual and his/her supports to record measurable progress toward recovery. Visual markers enable the individual to map progress, so he/she is able to look back at achievements with pride, while also visualizing the recovery path that lies ahead. 

For professionals and provider agencies, the Course of Recovery demonstrates medical necessity by creating a clear path from each recorded activity back to the individual’s stated barriers, goals, and diagnosis.

The Wellness Plan is a method for developing plans for individuals who do not have an established mental health diagnosis. These are people who may find themselves struggling with their overall quality of life and mental, emotional, or physical wellbeing. The Wellness Plan is a useful tool for creating realistic paths to positive change by articulating goals and identifying life domains that could be improved with targeted skill development.

  • Person Centeredness is CORE’s guiding principle. A person-centered approach recognizes the individual as the expert on his or her own life. This respects the individual as a unique, dynamic and multi-faceted person, not solely defined by a mental or physical health diagnosis. Person Centeredness challenges the individual to take control of the recovery process and to lead his/her support team. This approach asks the individual to paint a picture of what his or her life is like now, what is and is not working, and what he/she hopes life will look like in the future. The Person Centered Approach is empowering and helps an individual realize and call upon the strengths he/she will use to initiate and sustain recovery.

  • A Person-Centered Plan is a tool that helps persons-served describe what their lives looks like now, what needs to remain the same, what needs to change, where they would like to be in the future and the supports needed to get there. Person Centered Planning helps clients and their natural supports guide change within their lives. Persons-served direct who, what, when and where services will be offered.
