The CORE Wellness Plan offers a straightforward process for setting goals and creating effective plans to achieve whole health. This leads professionals, and persons-served, to seek advice online, where they encounter conflicting and unreliable information.

  • The Wellness Plan

    A distinct tool on CORE's digital health platform, caters to individuals with or without mental health diagnoses. It guides professionals and persons-served through conversations about personal strengths and helps set long and short-term goals, empowering them to embark on their wellness journey with confidence.

  • Barrier Groups

    A crucial element of the CORE Wellness Plan, helping individuals understand and identify obstacles hindering progress toward their goals. The plan presents 20 Barrier Groups, addressing a wide array of challenges that could affect anyone.

  • CORE Activities

    After identifying barriers, the Wellness Plan suggests curated lists of CORE Activities. These activities introduce concepts and skills that will help individuals achieve goals and ultimately improve their overall health and wellness.

  • The Wellness Plan was specially designed to support health and human service professionals that work in any capacity to improve quality of life for the people they serve. While the Wellness Plan is a helpful resource for persons-served, it is meant to guide and enhance the important work done by “helping professionals.” 

  • The Wellness Plan and Course of Recovery (COR) are similar – however, the COR requires the selection of one or more diagnoses. At the Barrier Group stage in the COR, the lists of barriers and Barrier Groups are specific to the selected diagnoses. The Wellness Plan does not require the person-served to identify a diagnosis and offers a selection of 20 Barrier Groups that represent common challenges.

  • Not right now. CORE recognizes that the Wellness Plan could serve as a useful resource for self-directed plans for positive change. At the moment, however, the Wellness Plan is accessed as part of a full license to CORE’s digital health platform, meant for use by a wide variety of healthcare professionals.
