Collaborative Documentation
Percensys CORE Learning promotes the use of an efficient and accurate way to document all necessary elements of a service note while continuing to provide services directly to the individual served. Goal specific discussions of effectiveness and perceived benefits of activities at the conclusion of sessions:
Increase the individual’s sense of empowerment by fully involving him/her in the process of evaluating services.
Reinforce skills taught during the session.
The collaborative review process helps professionals to continue to “plant seeds.”
Reinforce the notion that all sessions are purposeful and planned, not spontaneous or casual.
And more. . . .
Experience Based & Evidence Informed Practices
CORE skill-building interventions were written by professionals from multiple disciplines with varied roles and credentials. Each author drew upon their own experience delivering and managing behavioral health services. This development team utilized their knowledge and experience implementing evidence based practices and applied it to the development of CORE activities. Many activities are reflective of practices from Wellness Management & Recovery, Cognitive Behavioral Therapy, WRAP, Dialectical Behavior Therapy and Motivational Interviewing.
