Treatment Planning

Person Centeredness is our company’s guiding principle. A person centered approach recognizes the individual as the expert on her own life. This respects the individual as a unique person, not defined by a mental health diagnosis. Person Centeredness challenges the individual to take control of the recovery process and to lead her support team. This approach asks the individual to paint a picture of what her life is like now, what is and is not working, and what she hopes life will look like in the future. The Person Centered Approach is empowering and helps an individual realize and call upon the strengths she will use to initiate and sustain recovery.

Person Centered Planning

Percensys CORE Learning helps professionals apply person-centered principles during the development of Person Centered Plans (PCP) for recovery. This type of service planning is driven by the individual, and is directed by findings from the comprehensive clinical assessment and the determination of the individual’s mental health diagnosis. These plans do not command ways to “fix” a person, but challenge the individual, supporting professionals and others to understand what is important to and for the individual, his goals and aspirations and the barriers that have prevented him from reaching these goals. This approach gives everyone involved a better understanding of how barriers have uniquely impacted the individual.

Overcoming Barriers

In the context of mental health recovery, a barrier is anything that prevents a person from reaching their recovery goals. Barriers inhibit individuals from functioning as independently as possible and may cause individuals to neglect tasks crucial to their health and well-being. Percensys CORE Learning helps individuals and their supporting professionals identify specific barriers that interfere with goal attainment. Identifying barriers can provide clarity during development of treatment plans. This process is true to the spirit of person-centeredness, because it actively engages individuals in reviewing and selecting intervention activities. This increases treatment transparency and reduces anxiety individuals sometimes experience when beginning services.

Course of Recovery Tool

Percensys CORE Learning’s Course of Recovery Tool aids individuals, natural supports and professionals in the process of building and following a Person Centered Plan. The tool helps the individual maintain an accomplishment driven life by providing a framework for identifying goals and useful, rehabilitative activities. Designed as a quick reference, the Course of Recovery is a user friendly way for the individual and his/her supports to keep record of measurable progress toward recovery. Visual markers enable the individual to map his progress so he is able to look back at achievements with pride, while also visualizing the recovery path that lies ahead.
